Threshold Community Midwives is a home birth midwifery practice in Central Vermont led by a team of two midwives - Emmy Nadav, CPM, LM and Tessa Landreau-Grasmuck, CPM, LM, NHCM. We offer holistic, grounded, personalized and empowering reproductive / generative healthcare for all genders and families. We believe that all people should have access to a generous, loving, autonomous, and dignified entrance into the world and transformation within it. We are dedicated to building trusting relationships and treating you with compassion and dignity. We honor the body’s inherent power and wisdom to make it through life’s transformations and cross through the thresholds. 


As midwives we are experts in the processes of fertility, pregnancy, birth and postpartum. Our services span the childbearing cycle, ranging from holistic fertility care, including fertility consultations and home intrauterine insemination (IUI), comprehensive and personalized prenatal care, continuous hands-on support for labor and birth, postpartum care for the parent and newborn, including lactation support. We also offer holistic care and counseling for sexual, pelvic, reproductive and menstrual cycle health.


We welcome all people, genders, sexualities and family structures into our practice. We are committed to providing accessible, culturally attuned care to people of any of race, ethnicity, color, body size, religious or spiritual belief, cultural background, or available economic resources.


About Tessa:

Tessa Landreau-Grasmuck (they/she) is a Certified Professional Midwife (CPM) and a Licensed Midwife (LM) in the state of Vermont and a New Hampshire Certified Midwife (NHCM).


Tessa was born into the hands of a midwife at a freestanding birth center in Philadelphia. Tessa has been a full-spectrum birthworker and reproductive justice organizer for 15+ years. Tessa graduated from Birthwise Midwifery School in 2021, and has attended over 100 births and countless other pregnancy outcomes at homes, birth centers and hospitals around the country. As a queer provider, Tessa is especially committed to working with queer and trans folks across all reproductive experiences.
